昨日の書込みの続きです。
今日の午前中は、茨城県福祉相談センターに療育手帳判定時の各種検査結果について「証明書」の発行をしてもらえるか照会をしました。
障害基礎年金の裁定請求手続中であることを伝え、各種検査結果の証明書の発行をしてもらいと伝えると、以前にも同様な証明書を発行したようでなんなく了解をいただきました。
ただし、「証明書発行願」を提出してほしいとのことでした。
今日の午後、直接センターに伺って手続をする旨伝えました。
午後には、「医療相談」時に持参してほしいという「障害診断書等用紙一式」を貰うため、市役所の国民年金課の窓口に行きました。
窓口では、障害基礎年金の裁定請求をするための用紙一式がほしい旨伝えると、「初めての相談ですか」と言われたので、以前に相談している旨を伝えると「相談記録簿」を確認していました。
確かに、以前「精神の障害用」の診断書用紙を貰いに行ったとき相談した記憶がありました。
その相談記録簿に以前の相談概要が記録されていました。
医療相談に行く時間の関係もあり、「とにかく、診断書など一式書類を貰いたい」と身構えて言ったのですが、「とりあえず、中に入ってください」と言われたので、言われるままに事務室の椅子に座ると「診断書（精神の障害用）」と「（病歴・就労状況）申立書」、そして「裁定請求時に必要な書類一覧」を持ってきてくれ順序良く丁寧に説明をしてくれました。
そのおじさんは、申立書の書き方の説明書も持ってきてくれ、さらに丁寧に説明をしてくれました。
またまた、もしかしたら、用紙が足らないかもしれないと予備の申立書をもう１枚持ってきてくれました。
なお、裁定請求書は診断書や申立書、添付書類が揃ってから、窓口で説明をするのでそのときに書きましょうと言われました。
確かに、生年月日後３ヶ月以内に手続をすればいいので、まだまだ時間（生年月日まであと２ヶ月）はあります。
（対応してくれたおじさん、ありがとうございました）（身構えて話していた自分が恥ずかしかったです）
診断書等用紙一式が揃ったので、１４時からの医療相談のクリニックに向かいました。
クリニックには１０分前には着いたのですが、とりあえず受付をと考え受付を済ませ３階の待合室で待っていました。
ほどなく、名前が呼ばれ、「相談室」での医療相談です。
対応者は、精神保健福祉士であるソーシャルワーカーが医療相談ということで、障害年金の概要から説明をしていただきました。
やはり、認定精神科医が「診断書」を書く前の事前準備（病歴・就労状況申立書＝本人または代筆者が記入と診断書を書くに当たっての関係資料の準備）でした。
ソーシャルワーカーに持参した「母子手帳」、母子手帳の内容を補完する「誕生から現在までの生い立ち」、「療育手帳」、障害者自立支援法による「受給者証」、それから「診断書（精神の障害用）・申立書の用紙」を提示しました。
そのときに、認定精神科医が診断書を書くに当たって、心理テストやＩＱテストの結果などの各種検査結果を県福祉相談センターから「証明書」を出してもらう予定である旨伝えました。
その証明証を出してもらうに当たって、「一番近い判定時の検査結果」が良いのか「初回から前回までの検査結果」で良いのか確認したら、やはり「初回から一番近い判定時に使用した各種検査結果」を提出してもらいたいとのことでした。
その後は、娘の誕生から小学部入学前までの生い立ちやその時分の出来事を詳細に説明することになりました。
でも、誕生からの生い立ちを記録したものを持っていきましたので、すんなりと（笑いも交えながら）説明することができました。
相談開始から４５分頃には、娘が立ったり座ったりの行動をし始め落ち着かない素振りだったので、ソーシャルワーカーさんが「あと、５分くらいだよ」と言って、終わり間近を予告してくれました。
次回は、小学部入学以降現在までの状況を説明してくださいということで、本人は一緒に来なくともいいですよということになりました。
１階の受付で、ソーシャルワーカーさんが「医療相談予約簿」を見てくださり、次回の予約（次週の木曜日）をして終了でした。
診断書作成のために認定精神科医の診断もする必要があり、それは誕生日以降になります。
ちなみに、医療相談費用は健康保険証と〇福受給者証で窓口負担はありませんでした。
なお、診断書料は15,750円だそうです。
（社会保険事務所からの照会があった場合は文書料として2,100円かかるようです）
医療相談も終わり両親の説明に付き合ってくれたので、ご褒美にクリニックの入口の自動販売機で自分で好きな飲み物を自分でお金を投入して買い、娘は美味しそうに飲んでいました。
（自閉症の娘は「自分で好きな飲み物を選んで、自分の財布からお金を出してもらって、自分で自動販売機の投入口にお金を入れて、好きな飲み物のボタンを押してから、好きな飲み物が出てくる」という一連の行為が気に入っています）
そのあとは、県福祉相談センター（三の丸庁舎２階）に行って、証明書発行手続きをしました。
担当者は相談室に案内してくださり、証明書発行願いを書き認印を押印して手続は終了ですす。
なお、療育手帳の更新手続が１１月２日に予約をしている旨説明をしたところ、証明書を渡すのはそのときにでもいいかということだったので、次回の医療相談日に確認をしてその旨連絡をすることにしました。
いろいろと勉強になった一日でした。
とはいうものの、昨日のブログに書き込んだサイトで事前勉強をしておいて良かったと思っています。
